Disaster Planning must be Risk-based to be effective in dealing with the emergency situations. The book “ Nothing is Perfect” discusses Risk-based Disaster Planning using different methods for identification and quantification of different disaster scenarios both onsite and offsite. Chapters included are failure analysis of safety critical equipment used as barriers from accident and various analysis methods, Root Cause Analysis techniques, scenario modeling for fire, explosion and toxic emergencies and various consequence models like BLEVE, fire, explosion, toxic effects. Also added special chapters on risk from liquefied gases, rescue and evacuation planning based on the use of crisis/evacuation mappings; transport emergency planning, disaster recovery process and business continuity planning. Lessons leaned from mock-drills and actual disaster case studies are highlighted. Dr U K Chakrabarty, PhD is a highly qualified Safety professional and aclaimed Trainer in the domain of Oil & Gas Industry with 36 years of experience. A Graduate Chemical Engineer and a PG degree in Industrial Safety Eng. holding a PhD in Chemical Eng. with specialisation in Risk Assessment. Int.Dip in Environment Management from British Safety Council and a member of ASSP. Worked in KNPC, Kuwait as Safety Engineer. Authored more than 100 technical papers published, some are in Elsevier journals and three books in Industrial Safety and Disaster Management. Awarded with ‘Apex India Lifetime Achievement’, and ‘National BBS Ambassador’ and ‘Safety Man of the Year’ Awards.
